RFQ Form Fixes — Developer Reference
This prototype demonstrates the requested behaviors. Implement equivalent JS and HTML changes in the Interchange CMS template at /c/affil/rfq2.html. Existing brand styles, form fields, and CMS tags should be preserved.
Fixes included: (1) Category selector with fade + show/hide specs, (2) Dynamic step indicator, (3) On Demand vs. quantity mutex, (4) Phone field optional with format hint, (5) "How did you hear about us?" dropdown options, (6) Privacy Policy note above submit. Removed: "How It Works" button (does not belong on a form page with its own step indicator).
